Use this page only if the integrated Workshop Check-In is unavailable on training day.

Facilitators: share that link (or the direct Google Form URL below) at the door when D1 check-in is down. Trainees still need normal Workshop Access login for My Workshop and lab content — this form is attendance only.

Facilitator runbook (setup + training day): check-in-fallback-guide.html — open locally from the repo or on the deployed site under trainer/.

Complete Name, Email, and optional Department so attendance is recorded.
